A case manager assists individuals with meeting their immediate needs across all life domains including housing, income, education, mental health, socialization, ADL’s, drug and alcohol, and physical health by linking them with community resources to accomplish their goals and needs in these areas. The Blended Case Management program strives to provide support to individuals to help foster independence and maintain their recovery in the community setting. Services are delivered in an individual’s home, our outpatient offices, or in the community.
